WHAT TO ANTICIPATE


Your initial examination at Waukee Dental is scheduled for 1 hour and 30 minutes.  This allows our Waukee Dental Staff the opportunity to take extra time to get to know more about you, your health history and your dental history.  Your first appointment will include paperwork, a series of dental x-rays for proper diagnosis of anterior (front) and posterior (back) teeth,  charting, periodontal probing, TMJ exam, oral cancer screening, visual examination, diagnosis and treatment recommendations.  If there is a time remaining during your scheduled appointment time your new dental hygienist will perform a dental cleaning.

Our goal for every new patient experience is to make you feel comfortable and right at home in our dental office while completing a through and complete examination.  Following the examination, we will review our findings and provide you with an explanation of your oral health and what we can do to help you.

What to bring:

  • Your Dental Insurance Card
  • Your Drivers License
  • Any x-rays from any previous dentist from the past year
  • Patient Information Forms

New Patient Forms

Paperwork can be exhausting.  We have posted our new patient information forms online to make your new patient experience at Waukee Dental as stress-free as possible.  Please click the link below to submit your paperwork to our front office staff prior to your dental appointment.  No time to complete the forms before your visit? No problem, we just ask you to arrive 15-20 minutes early to allow time to complete these prior to your scheduled appointment time.

If you have x-rays or dental records you would like transferred to our office, please fill out the Transfer Request form below and email that to waukeedental@securepracticemail.com and we will be happy to assist you with that process.
