Your initial examination will include a visual examination, charting, periodontal probing, TMJ exam, oral cancer screening, dental cleaning, diagnosis and treatment recommendations, and a series of x-rays for proper diagnosis of anterior (front) and posterior (back) teeth.
Please arrive 15-20 minutes before your appointment to complete our new patient forms or, download them from our website and fill them out ahead of time.
Dental Records & X-rays
Your previous history with dental treatment will assist your new dentist and team in determining your current level of treatment. If possible, make arrangements for these documents to be transferred to the new practice.

- Symptoms Do you have any pain or sensitivity in your mouth, gums or teeth?
- Gag Reflex If you have a very sensitive gag reflex, you probably have had difficulty with dental treatments. If the dental team is aware of this, they will be able to avoid certain sensitive areas.
